Complete ACH form HFS3881G and email to HFS.GEMT@Illinois.gov (preferred) or mail check noting 'GEMT Payment' and invoice number to Illinois Department of Healthcare and Family Services, Bureau of Fiscal Operations - GEMT, PO Box 19491, Springfield, IL 62794-9491.
