Summary
The Illinois Department of Healthcare and Family Services is launching a Learning Collaborative for healthcare providers interested in joining the federal Certified Community Behavioral Health Clinic (CCBHC) Demonstration Year 2. Eligible behavioral health providers must apply by April 4, 2025 to participate in mandatory monthly training sessions before becoming certified CCBHC sites.

By April 4, 2025, 11:59 p.m. Central Time: Behavioral health providers interested in CCBHC certification must complete and submit the Learning Collaborative application through the online Qualtrics Survey at https://hfs.illinois.gov/medicalproviders/certifiedcommunitybasedhealthcenterinitiative.html. Application must be submitted by an individual authorized to sign attestations for the organization. No extensions will be granted.
