Ensure all applications are submitted through one of four approved methods: HFS Application Agent account, HFS Long Term Care Provider account, individual resident accounts with approved representative consent, or LTC Provider Portal functionality. Contact Bureau of Long Term Care at 1-844-528-8444 for account setup assistance. Applications submitted incorrectly will be delayed in processing.
