Summary
Illinois has extended the deadline for Long Term Care facility cost reports from 90 days to November 30 for facilities with fiscal years ending on or before June 30, and 5 months for facilities with fiscal years ending after June 30. For 2024 reports, facilities with December 31 fiscal year ends must submit by May 31, 2025.
Action Required
By May 31, 2025: Long Term Care facilities with December 31, 2024 fiscal year ends must submit 2024 cost reports to HFS.HealthFinance@illinois.gov electronically and mail signed paper copy to Bureau of Health Finance in Springfield. Ensure Ownership Listing pages are 100% complete including middle initials for all owners.
