Illinois is requiring publicly owned ambulance providers to submit new cost reports by October 1, 2025 to participate in the 2026 Ground Emergency Medical Transportation (GEMT) supplemental payment program. New participants must also submit signed Intergovernmental Agreements.

By October 1, 2025: Publicly owned ambulance providers in Illinois must submit cost reports (both Excel and PDF with signatures) and signed IGAs (for new participants) to HFS.GEMT@illinois.gov to participate in 2026 GEMT supplemental payments. Contact authorized government officers (Mayor, Administrator, or Fire District head) to sign IGAs if participating for first time.
