Summary
Illinois Department of Healthcare and Family Services is notifying publicly owned Ground Emergency Medical Transportation (GEMT) providers that payments for FY 2025 Quarters 1-3 (July 1, 2024 through March 31, 2025) are due by August 31, 2025. Providers can pay via ACH Direct Debit (preferred method) by completing form 3881G or by check.

By August 31, 2025: Publicly owned GEMT transportation providers must submit payment for FY 2025 Quarters 1-3. Complete form 3881G and email to HFS.GEMT@Illinois.gov for ACH payment (preferred), or mail check noting 'GEMT Payment' with invoice number to Illinois Department of Healthcare and Family Services, Bureau of Fiscal Operations - GEMT, PO Box 19491, Springfield, IL 62794-9491.
