Summary
Illinois Supportive Living Program providers must complete conversion screenings through the Care Coordination Unit when residents transition from private pay to Medicaid. The screening must be completed before the Medicaid conversion can occur, and late screenings will result in adjusted admission dates.
Action Required
Immediately: SLP providers must request conversion screenings from the Care Coordination Unit (CCU) before transitioning any resident from private pay to Medicaid. Contact the Bureau of Long-Term Care at HFS.SLF@illinois.gov or 1-844-528-8444 for questions. Failure to complete screening prior to conversion will result in admission date adjustments to reflect the screening date.
