Summary
Illinois now requires privately owned ambulance providers to submit annual cost reports to the Department of Healthcare and Family Services starting with 2026 data. Cost reports must be submitted within 5 months after the data collection period ends, but no later than May 31, 2027.
Action Required
By May 31, 2027: Private ambulance providers must complete and submit annual cost reports covering 12-month period (Jan 1-Dec 31, 2026 or fiscal year overlapping 2025-2026) to HFS.PrivateAmbulance@illinois.gov in both Excel and signed PDF formats. Use cost report template from Illinois HFS Cost Report webpage. Failure to submit may affect provider enrollment status with the Department.
