Summary
Illinois Medicaid requires all Supportive Living Program (SLP) providers to have residents re-sign their contracts/leases annually. If any changes were made to the contract, residents must provide a new signature acknowledging the updated terms.
Action Required
SLP providers must implement annual contract re-signing process for all residents. If contract unchanged from previous year, resident may sign beneath prior signature. If any changes made, clearly note changes and obtain new resident signature with date. Refer to 89 Ill. Adm. Code Section 146.240 for compliance requirements.
