Summary
Maryland Medicaid hospice providers must submit new provider applications for each of the 6 designated regions where they provide routine home care and continuous home care services. Each region will receive a unique provider number that must be used on UB-92 claim forms based on the service location.
Action Required
By July 21, 1999: Submit new hospice provider application packets for each Maryland region where services are provided. By September 1, 1999: Begin using exclusively the new region-specific provider numbers on UB-92 claim forms in block 51 based on service location. Billing team must update systems to use correct regional provider numbers or claims will be rejected after September 1999.
