Summary
NC Medicaid is ending the Community/Behavioral Health service under taxonomy 251S00000X on April 20, 2027. Providers must enroll in a new Innovations Waiver Service and report national accreditation (if required) to continue receiving reimbursement for Innovations Waiver services.
Action Required
Before April 20, 2027: Innovations Waiver providers with taxonomy 251S00000X must complete an NCTracks Manage Change Request (MCR) to select a new Innovations Waiver Service and report national accreditation if required. Check the Provider Permission Matrix for available services. Failure to enroll will result in loss of eligibility for Innovations Waiver reimbursement.
