By May 1, 2001: Prescribers must register all clozapine patients with either Zenith-Goldline (1-800-507-8334) or Mylan (1-800-843-9915) registry. Pharmacies must register as distribution centers with the chosen generic company and consistently dispense the same manufacturer's product for each patient. All blood work must be sent to the patient's registered manufacturer. Update billing systems to reflect new reimbursement rates: $2.50 per 100mg tablet and $1.00 per 25mg tablet plus dispensing fee.
