Before your revalidation cycle end date: Check email addresses are current in IMPACT system (allows up to 3 addresses). Wait for 90-day and 30-day email notifications from Illinois Department of Healthcare and Family Services before attempting revalidation. Complete revalidation for each service location separately through IMPACT system to maintain enrollment and payment eligibility. Contact Provider Enrollment at 877-782-5565 if assistance needed. Failure to revalidate will result in enrollment termination and loss of payment eligibility with no retroactive enrollment.
