Maryland Medicaid will now accept Medical Eligibility Review Forms (DHMH 3871) signed by either physicians OR nurse practitioners for nursing facility level of care determinations. Previously, only physician signatures were accepted for these eligibility assessments.

Immediately: Update billing team procedures to accept Medical Eligibility Review Forms (DHMH 3871) signed by nurse practitioners for nursing facility level of care determinations in Maryland Medicaid cases. Train staff that both physician and nurse practitioner signatures are now valid for these forms. Note: PACE, chronic, rehabilitation and specialty hospital levels of care still require physician signatures only.
