Summary

Maryland Medicaid MCOs must report provider contract terminations affecting 50+ members to the Department at least 30 days prior to termination, including member transition plans and continuity of care processes. For terminations affecting 100+ members, a completed Contract Termination Survey is also required.

At least 30 days before provider contract termination (or within 5 days if less notice given): MCOs must submit written notice to DHMQA including termination date, affected providers, member count, and transition plan with member letters, alternative provider lists, and continuity of care processes. For 100+ affected members, complete Contract Termination Survey with required Excel spreadsheets.
