Summary
Maryland's Waiver for Older Adults program requires preauthorization forms for Environmental Accessibility Adaptations, Assistive Devices, Environmental Modifications, and Assistive Equipment services. All claims for these services must be submitted on paper with signed preauthorization forms attached, and electronic claims will not be processed.
Action Required
By June 25, 2003: Billing team must obtain signed preauthorization forms from case managers before providing GO207, GO214, GO218, and GO219 services. Submit only paper DHMH 248 claims (or HCFA 1500 for Assistive Devices) with preauthorization forms attached to DHMH P.O. Box 1935, Baltimore, MD 21203. Do not submit electronic claims as they will not be processed without preauthorization forms.
