Summary
Maryland Medical Assistance (Medicaid) has streamlined the ownership transfer process for medical day care centers. New owners no longer need to recertify nursing facility level of care for existing patients when purchasing a facility, but must still complete standard provider enrollment applications.

Maryland medical day care centers undergoing ownership changes: New owners must complete Medical Care Program Provider Application and submit to Division of Provider Services. Include tax ID or SSN, Provider Ownership and Control Disclosure Form, and Provider Agreement. Attend mandatory Office of Health Care Quality Provider Orientation (schedule at 410-402-8135).
