By November 1, 2006: Providers serving Waiver for Older Adults and Living at Home Waiver participants must attach new standardized forms (DHMH 4658 series) to all timesheets for payment. Download forms from www.dhmh.state.md.us or www.mdoa.state.md.us, or obtain paper copies from Area Agencies on Aging after October 15, 2006. Complete required training sessions scheduled for September 29 and October 10, 2006. Incomplete or incorrect forms will be returned unpaid.
