Summary
Blue Cross and Blue Shield of Texas is excluding 15+ brand-name drugs from coverage and moving them to generic alternatives effective April 1, 2025. Changes apply to Balanced, Performance, Performance Annual, and Performance Select drug lists. Members will receive notification letters. This affects pharmacy benefits and may require prior authorization updates for affected conditions (opioid withdrawal, seizures, rosacea, cancer, erectile dysfunction, hereditary angioedema, neutropenia).

By March 15, 2025: Billing and clinical teams must review the complete drug exclusion list and identify patients currently prescribed excluded medications (LUCEMYRA, ORACEA, OXTELLAR XR, PREVIDENT RINSE, PROMETHAZINE VC/CODEINE, SPRYCEL, STENDRA, TAZORAC, ZIEXTENZO, SAJAZIR, and generic promethazine-phenylephrine syrup variants). Update pharmacy benefit verification processes in billing software to flag these excluded drugs. Educate front desk and clinical staff to proactively contact affected patients before April 1, 2025, to discuss generic alternatives provided by insurance. Coordinate with providers to transition prescriptions to covered alternatives listed in the policy (e.g., doxycycline hyclate for ORACEA, generic dasatinib for SPRYCEL, generic avanafil for STENDRA). Monitor claims submitted after April 1, 2025, for these drugs—they will be denied as non-covered. Establish process to request prior authorization exceptions if clinically necessary. Note: Changes do not apply to Basic Annual, Multi-Tier Basic Annual, Enhanced Annual, Multi-Tier Enhanced Annual, or Performance Annual plans until January 1, 2026.
