Summary

Maryland's Money Follows the Person (MFP) Demonstration program is now designated as the Local Contact Agency for processing MDS 3.0 Section Q referrals from nursing facilities. Nursing homes must refer residents who indicate interest in community-based services to MFP within 10 business days using the provided MFP Data Sheet form. All residents, regardless of payer status, are eligible for Program Education services provided by local Area Agencies on Aging.

By October 1, 2010: Nursing home billing and administrative staff must implement the new MDS 3.0 Section Q referral process. Establish a workflow to identify Section Q referrals (Items Q0500 or Q0600) and submit completed MFP Data Sheets to (410) 333-5333 within 10 business days of MDS completion. Ensure all required fields are completed (minimum: resident name, address, phone, birth date, SSN or MA number). Train staff on the referral timeline and tracking process.
