Summary
CMS has updated the Important Message from Medicare and Detailed Notice of Discharge forms that hospitals must provide to Medicare beneficiaries. Hospitals must transition to using the new versions by May 15, 2026, and the updated forms are effective until March 31, 2029.
Action Required
By May 15, 2026: Hospital billing and patient services teams must download and implement the updated Important Message from Medicare and Detailed Notice of Discharge forms from CMS website (available in English and Spanish). Replace current forms in patient packets and discharge processes. All Medicare inpatients must receive the Important Message form, and the Detailed Notice should only be provided if a beneficiary requests an appeal.
