Action needed
Immediately: Billing team must ensure preauthorization is obtained from Telligen through Qualitrac before scheduling cochlear implant or auditory osseointegrated device surgeries. Update billing system to use revenue codes 0470, 0471, 0472, or 0479 for audiology services and include HCPCS codes L8614 (cochlear implant) or L8690 (auditory osseointegrated device) in Form Locator 44 on outpatient UB04 claims. Failure to obtain preauthorization may result in fund recoupment by the Department.
