Summary
Starting August 1, 2026, new providers joining the network must submit a W-9 form with both legal name and DBA name (if applicable), plus official IRS documentation (SS-4, 147C forms, or other IRS Tax ID notification) as part of their onboarding application. Current in-network providers are not affected unless they have organizational changes.
Action Required
Before August 1, 2026: Practice administrators should ensure any new provider applications include updated W-9 forms with both legal and DBA names plus required IRS documentation (SS-4 or 147C forms). Current providers need no action unless there are name or TIN changes requiring new onboarding applications.
