Update rates for device codes J7296, J7297, J7298, J7300, J7301, and J7307, and procedure codes 58300, 58301, 11981, 11982, and 11983. Apply different facility and non-facility rates for procedures 58301, 11982, and 11983 based on setting. Update encounter forms and billing software rules to ensure new rates are applied to all LARC claims processed on or after February 15, 2024. Verify all rate changes are active in the system before the effective date. Failure to update rates will result in underbilling and revenue loss for Maryland Medicaid LARC services.
