By July 12, 2024 (or immediately upon discovery): Nursing home billing/administrative staff must (1) Determine whether your facility is among the five facilities with the highest Medicaid patient days in FY 2024 to apply the correct assessment rate ($33.21 or $2.72 per non-Medicare patient day); (2) Register and access the Cognito Forms reporting tool at https://www.cognitoforms.com/MDH3/NursingFacilityQualityAssessmentReport; (3) Review the Cognito training instructions at the provided URL; (4) Calculate total non-Medicare patient days for FY 2025 and multiply by the applicable assessment rate; (5) Submit reports via the Cognito tool; (6) Await payment remittance instructions from Myers & Stauffer LC's secure portal and process payments accordingly.
