Designate an authorized official to manage account access. By September 3, 2024: All PASRR Level I submissions must be made through the Qualitrac electronic questionnaire instead of paper forms. Update admission workflows to require attending physician digital signatures for exempt hospital discharges before submission can be processed. Download PASRR Level I results from Qualitrac and contact the Local Health Department to request Level II assessments when indicated. Failure to transition to the electronic system may result in delayed PASRR determinations and admission delays.
