Summary
Maryland MCOs must screen all HealthChoice (Medicaid managed care) members for health-related social needs (HRSN) using a standardized MDH-approved tool at least once annually, effective July 1, 2025. MCOs must share collected HRSN data with CRISP by January 1, 2026, for population health analysis and care coordination purposes.
Action Required
By July 1, 2025: Clinical and administrative staff must implement HRSN screening workflow using the MDH-approved PRAPARE tool for all HealthChoice members at least once annually. Providers and intake staff should integrate screening questions into patient encounters (initial visits, annual wellness visits, or care coordination contacts) and document responses in EMR. By January 1, 2026: Billing/IT team must ensure HRSN data collected is formatted and transmitted weekly to CRISP via secure file transfer protocol (SFTP) and coordinate with MDH on data sharing requirements. All providers with HealthChoice panels should be informed that they may access HRSN data via CRISP to avoid duplicate screening. Failure to comply may result in MCO non-compliance with state requirements and NCQA accreditation standards.
