Summary

Maryland Medicaid hospice providers must now upload required documentation directly to the Medicaid Hospice Request Form instead of just attesting that documents exist in their records. All required federal and state hospice documentation must be uploaded with each form submission.

By July 1, 2025: Billing team must update hospice enrollment workflows to upload all required documentation (Election of Hospice Benefit, Certification of Terminal Illness, Changes in Hospice Provider, Revocation/Termination forms, and Notice of Eligibility) directly to the Medicaid Hospice Request Form at time of submission. Train staff on new upload requirements using available training materials. Simple attestation will no longer be accepted.
