Summary
Medicare providers participating in EmblemHealth's VIP Bold and Reserve Networks and/or ConnectiCare's Choice Network must complete and attest to Special Needs Plan Model of Care training by September 30, 2023. This is an annual CMS requirement for all dual-eligible Special Needs Plans.
Action Required
By Sept. 30, 2023: Providers who participate in EmblemHealth's VIP Bold and Reserve Networks and/or ConnectiCare's Choice Network must complete SNP Model of Care training and submit attestation at emblemhealth.com/snp-moc or connecticare.com/snp-moc. Failure to complete may affect participation in dual-eligible Special Needs Plans.
