By Aug 31, 2023: Licensed Home Care Services Agencies and Consumer Directed Personal Assistance Program Fiscal Intermediaries contracted with EmblemHealth must submit Department of Labor form LS300 and independently audited financial statements for 2021-2022 to wageparity@emblemhealth.com. Submit separate documents if contracted as both LHCSA and FI.
