Summary
Medicare providers participating in EmblemHealth's VIP Bold and Reserve Networks and/or ConnectiCare's Choice Network must attest to completing Special Needs Plan (SNP) Model of Care training by September 30, 2023. This is an annual CMS requirement for all providers serving dual-eligible SNP members.
Action Required
By September 30, 2023: Providers in EmblemHealth's VIP Bold and Reserve Networks and/or ConnectiCare's Choice Network must complete and attest to the combined 2023 EmblemHealth and ConnectiCare SNP Model of Care training. Submit attestation to receive confirmation of completion. Failure to complete may affect participation in dual-eligible SNP networks.
