Summary
EmblemHealth and ConnectiCare have permanently disconnected their fax lines for preauthorization requests and concurrent reviews as of May 1, 2024. All preauthorization submissions and supporting documentation must now be submitted through their provider portal instead of fax.
Action Required
Immediately: Stop using fax for all preauthorization requests and concurrent reviews. Billing team and clinical staff must transition to using EmblemHealth/ConnectiCare provider portal for all submissions. If practice doesn't have portal access, Administrator/Office Manager must complete Portal Account Request Form and set up user accounts with Clinical Staff roles. Non-participating providers must call EmblemHealth at 866-447-9171 or ConnectiCare at 860-674-5850 for assistance. Failure to use portal will prevent preauthorization processing.
