Summary
EmblemHealth and ConnectiCare will maintain payment parity for telehealth visits in 2025 by continuing to use existing office visit codes with appropriate modifiers. CPT code G2012 is retired and replaced with CPT 98016 for brief virtual check-ins, but the new 2025 telehealth codes will not be accepted.
Action Required
Immediately: Billing team must stop using retired CPT code G2012 for brief virtual check-ins with established patients. Update billing system to use CPT code 98016 instead. Continue using existing office visit procedure codes with appropriate telehealth modifiers and place of service codes for all other telehealth visits. Do not use any new 2025 telehealth codes as they will be denied by EmblemHealth and ConnectiCare.
