Summary
Starting April 30, 2025, EmblemHealth will require all dental providers applying for credentialing to complete a CAQH credentialing application form. Providers without a CAQH number must register with the CAQH Provider Data Portal and ensure their application contains complete, current information with validated service and billing addresses.
Action Required
By April 30, 2025: Dental providers must obtain a CAQH number by registering with CAQH Provider Data Portal (call 888-599-1771 for assistance). Complete CAQH credentialing application with current information and validate accuracy of service and billing addresses before submission to EmblemHealth. Failure to use CAQH application will prevent credentialing with EmblemHealth.
