Summary
EmblemHealth updated the Consumer Directed Personal Assistance Program (CDPAP) Agreement in February 2025 to reflect the transition to Public Partnerships, LLC as the single Statewide Fiscal Intermediary. All Medicaid members participating in CDPAP must sign this updated Consumer Agreement and submit it to EmblemHealth's long term support services.

Immediately: Billing team must ensure all Medicaid members participating in CDPAP sign the updated Consumer Agreement (February 2025 version). Submit signed agreements to EmblemHealth LTSS Care Management by email or mail to EmblemHealth, 55 Water Street, New York, NY 10275-0718, Attn: LTSS Care Management. Required for current authorizations, new authorizations, changes to designated representatives, and annually.
