Summary
ConnectiCare was acquired by Molina Healthcare on February 1, 2025. Some ConnectiCare members can now see EmblemHealth providers depending on their benefit plan. Providers should continue following current ConnectiCare processes and check the 2025 Summary document to verify which plans have EmblemHealth/ConnectiCare reciprocity.
Action Required
Immediately: Billing team must review the 2025 Summary of Companies, Lines of Business, Networks, and Benefit Plans document to identify which ConnectiCare plans have EmblemHealth reciprocity (check last column). For members with reciprocal plans, continue using current ConnectiCare billing processes and procedures. Verify patient eligibility and benefits before scheduling appointments.
