Action Required

Action needed
By May 31, 2021: Check all service locations' revalidation dates in PROMISe system. Submit any overdue revalidation applications immediately using the staggered schedule. For future revalidations: Submit applications at least 60 days before due date to avoid closure of provider service locations in PROMISe. Monitor for 90-day and 30-day revalidation notices.
