Summary
Florida Blue will offer free telehealth and in-home health assessments through Emcara Health to commercial plan members with HHS-HCC conditions from August-December 2025. Assessment results will be shared with the member's PCP for follow-up care coordination.

August 2025: Primary care providers should expect to receive health assessment reports from Emcara Health for Florida Blue commercial members and follow up with patients as appropriate. No billing changes required as assessments are conducted by Emcara at no cost to members.
