Summary
Michigan is implementing a new MI Care Team Health Home benefit effective July 1, 2016, for Medicaid, Healthy Michigan Plan, and MIChild beneficiaries with depression/anxiety plus qualifying chronic conditions. Providers must obtain signed consent forms (MSA-1030 and DCH-3927) and verify beneficiaries have 'HHMICARE' benefit plan assignment in CHAMPS before rendering services.
Action Required
By July 1, 2016: Billing team must verify 'HHMICARE' benefit plan assignment in CHAMPS system before providing MI Care Team services to eligible beneficiaries. Providers must collect and store signed enrollment form (MSA-1030) and consent form (DCH-3927) in patient health records. Beneficiaries without proper assignment will not be eligible for MI Care Team payment.
