By July 1, 2016: All PDN providers must use revised MSA-0732 form (dated June 2015) for authorization requests - requests on old forms will be returned. Billing team must ensure all PDN service logs clearly document specific time worked for each beneficiary and date of service, maintain electronic or paper logs as part of medical record. Update workflows to obtain required signatures from PDN nurse/agency AND beneficiary's parent/guardian on MSA-0732. Coordinate with multiple PDN agencies to report monthly utilization hours to PRD before payment processing. Failure to maintain proper logs may result in recoupment of reimbursement.
