Summary
Michigan Medicaid now provides separate reimbursement for Long-Acting Reversible Contraception (LARC) devices when provided immediately postpartum in inpatient hospital settings, outside of the maternity DRG payment. Facilities must submit separate professional claims with place of service 21 and use appropriate HCPCS codes with NDCs.

Ensure claims include hospital as billing provider, practitioner as rendering provider, appropriate HCPCS Level II codes, and NDCs. Facilities in 340B program must use U6 modifier and bill actual acquisition cost.
