Facilities must submit separate professional claims with place of service 21 and use appropriate HCPCS codes with NDCs.

Action Required

Action needed
For Michigan facilities providing immediate postpartum LARC: Update billing workflows to submit separate professional claims with place of service 21 for LARC devices. Ensure claims include hospital as billing provider, practitioner as rendering provider, appropriate HCPCS Level II codes, and NDCs. Facilities in 340B program must use U6 modifier and bill actual acquisition cost.
