Action Required

Action needed
By October 1, 2018: Providers in Michigan PIHP Region 2 should verify beneficiary HHO benefit plan assignment in CHAMPS before rendering services. Ensure collection and storage of signed MDHHS-5515 consent forms for all OHH beneficiaries. Update billing workflows to document beneficiary consent and diagnostic eligibility in the WSA system. Only one OHH provider may be paid per beneficiary per month.
