Summary
Florida Blue is implementing the Advocate+ Pharmacy Match program through Free Market Health starting January 1, 2026, which changes how specialty pharmacy prescriptions are processed and where prior authorizations are sent. Self-administered specialty medication prescriptions must now be sent to FMH Pharmacy, and prior authorization requests will be reviewed by Prime Therapeutics instead of CVS Specialty.
Action Required
By January 1, 2026: Update prescription routing to send all self-administered specialty medications to FMH Pharmacy (fax: 1-833-998-4435, NCPDP: 6013914). Change prior authorization submissions from CVS Specialty to Prime Therapeutics via CoverMyMeds for self-administered drugs. Continue sending provider-administered drugs to CVS Specialty and behavioral health medications to Genoa Healthcare. Update EMR and workflow documentation with new pharmacy contact information and routing rules.
