Summary
Florida Blue now requires Service Facility NPI or Florida Blue Service Facility ID on all professional claims performed in facility settings (Place of Service codes 19, 21, 22, 24, 51, 52, 61). Claims with missing or incorrect facility information will be rejected when the system updates.
Action Required
Immediately: Billing team must update all professional claims performed in facilities to include Service Facility NPI or Florida Blue Service Facility ID. For electronic claims, enter in loop 2310C of 837P format. For paper claims, enter in CMS 1500 fields 32, 32a, and 32b. Contact facility billing departments or use NPPES directory to obtain correct facility NPIs. Do not use provider's own NPI - claims will be rejected when system updates.
