Effective September 1, 2020: For Michigan Medicaid beneficiaries who die before delivery of custom fabricated DMEPOS items, billing team should contact MDHHS Program Review Division at 800-622-0276 to request partial reimbursement. Submit documentation including original PA tracking number, beneficiary date of death, date provider learned of death, itemized invoice with materials/labor costs, salvage value deductions, and fabrication stage completion. This applies only to custom fabricated items, not prefabricated or off-the-shelf DMEPOS.
