Summary
Michigan is rescinding COVID-19 flexibilities for non-emergency medical transportation (NEMT) effective May 12, 2023, returning to pre-pandemic requirements including completion of DHS-5330 forms for special transportation and stricter prior authorization processes.
Action Required
By May 12, 2023: Billing team must ensure completion of Medical Verification for Transportation form (DHS-5330) for all special transportation, round-trip mileage over FFS rates, and out-of-community medical care when local care is available. Update prior authorization workflow to fax all requests to MDHHS Program Review Division at 517-241-7813 or email to MDHHS-PRD-NEMT@michigan.gov. Train staff that Medical Transportation Statement (MSA-4674) can only be completed via telephone for missing information, not as primary method.
