By January 1, 2024: Michigan practices serving Medicaid patients should identify if they want to provide CHW services. Contact MDHHS designated contractor to register qualified CHWs in the MI Medicaid CHW Registry before enrolling as Medicaid providers. Update workflows to document licensed provider recommendations for CHW services and ensure face-to-face delivery requirements are met.
