Summary
Michigan Medicaid is eliminating the requirement for telephone prior authorization requests for enteral formulas and supplies effective July 1, 2024. Providers must now complete written forms (BPHASA-2401) instead of calling the PACER line for initial and renewal requests.
Action Required
By July 1, 2024: Providers ordering enteral formulas/supplies for Michigan Medicaid patients must stop using the PACER telephone line (844-722-3764) for prior authorization. Instead, complete the new BPHASA-2401 Medical Justification for Enteral Therapy Form available at michigan.gov/medicaidproviders and send to medical suppliers with written orders. Medical suppliers must submit prior authorization requests through CHAMPS system or fax to 517-335-0075. Emergency authorizations for hospital discharges can still use verbal requests at 800-622-0276.
