Florida Blue expanded their specialty pharmacy network with the Advocate+ Pharmacy Match program effective January 2026. Self-administered specialty medication prescriptions must now be sent to FMH Pharmacy instead of directly to specialty pharmacies, while prior authorizations go through Prime Therapeutics via CoverMyMeds.

By January 1, 2026: Update specialty medication prescription routing - send all self-administered specialty prescriptions to FMH Pharmacy (Fax: 1-833-998-4435, NCPDP: 6013914) at 40 24th St. Suite 150, Pittsburgh, PA 15222. Submit self-administered prior authorizations through Prime Therapeutics via CoverMyMeds. Continue sending provider-administered orders to CVS Specialty. Update EMR templates and staff workflows for the new prescription routing process.
