Summary
Florida Blue now requires Service Facility NPI (or Florida Blue Service Facility ID for paper claims) on all professional claims billed in facility settings for Place of Service codes 19, 21, 22, 24, 51, 52, and 61. Claims will be rejected with error code AP0677 if the facility NPI is missing or incorrect.
Action Required
Immediately: Billing team must update all electronic claims to include Service Facility NPI in loop 2310C of 837P format for Place of Service codes 19, 21, 22, 24, 51, 52, and 61. For paper claims, include Service Facility NPI or Florida Blue Service Facility ID in CMS 1500 fields 32, 32a, and 32b. Verify facility participation status and obtain correct NPIs from facility billing departments or NPPES directory before claim submission. Claims with missing, invalid, or incorrect facility NPIs will be rejected with error AP0677.
